Personal Care Aides Near Me
Medicaid recipients can qualify for long-term home care through New York State’s Aged and Disabled Care System. Eligibility for this system includes Maximus nurse visit/telehealth visit. The family then selects a long-term care company that is responsible for all aspects of maintaining the patient’s health. Home care is delivered by Caring Professionals, a licensed home care service agency. Care professionals can help families through this entire process and provide ongoing support by explaining the necessary documents, assessments, and enrollment procedures:
The Projects That Are Breaking Ground In Patient Care
Medicare recipients may qualify for short-term home care and skilled services after a medical episode. These are arranged by the discharge planner (social worker) at the hospital or rehabilitation centre. The CHHA (Certified Home Health Agency) has overall responsibility for all services in the home, including occupational therapy, physical therapy, speech therapy, wound care, and social work. The Home Care Agency provides Home Health Aid (HHA) in coordination with the CHHA. Care professionals can help families navigate discharge home and ensure all necessary services are provided seamlessly.
Coordinate with CHHA referral to medical enrollment services if needed Refer to private pay home care services if needed
If you don’t have Medicaid, you may still qualify for this free wrap around coverage. Our intake specialists can direct you to a quality Medicaid enrollment service that does not charge for its service.
Please note: Caring professionals do not make decisions on application approvals or schedule home care hours. We provide only quality home care.

New York State Medicaid determines eligibility after an individual is approved for Medicaid. MLTC is responsible for the overall health and well-being of its members and operates as managed care. Once the MLTC is approved you can choose the MLTC of your choice to manage your health services and home care.
We accept managed long-term care plans. Contact us for complete list of all contracted plans.

Special Report: The Challenge Of Our Age
One of my family members or I is in the hospital and we are about to be discharged and we are looking for a home health aide to provide part of our short term care, can caring professionals help.
Yes Caring Professionals has many long-term relationships with various CHHAs in New York City and across the state, the people who organize your home care after you are discharged. Please ask us by name and we will be happy to provide an HHA for you or your family. If you need any advice or guidance contact us here
Not much. Both require current qualifications from New York State after a short period of training (about 75 hours) in an approved training program. A personal care assistant (PCA) has less training than a home health aide (HHA). An HHA may perform all of the primary care services that a PCA does. A home health aide can work on PCA cases, but PCA cannot work on cases specified for an HHA.
The MLTC specifies which services are required by the patient—PCA services or HHA services. Both PCAs and HHAs are staffed by caring professionals.
